Botox for the First Time: Everything Worth Knowing Before You Arrive

Deciding on a first aesthetic treatment often brings excitement—and natural worries. Will it hurt? Will I look “frozen”? How should I prepare, and what happens afterward?

As a physician who practices aesthetic medicine and guides many patients through their first injection, my goal is to make the process as transparent, calm, and safe as possible. Here is everything worth knowing before you arrive at the clinic.

How do you prepare for the first treatment?

Preparation for Botox is relatively simple, but a few small steps can make the visit smoother and reduce the risk of bruising:

  • Avoid blood-thinning painkillers — for about 3 days before treatment, avoid pain relievers that contain ibuprofen (such as Advil or Nurofen) and aspirin, which increase bruising tendency.
  • Avoid alcohol — ideally do not drink alcohol in the 24 hours before treatment.
  • Consultation — the first visit always starts with a consultation. We clarify what bothers you, assess facial muscle movement, and tailor approach and dose personally. The goal is to look refreshed—not “done.”

What happens in the treatment room? Does it hurt?

Pain is one of the most common questions. The good news: Botox injections are among the lighter and more comfortable aesthetic treatments.

After assessment, injection points are marked. The injection uses a very fine needle. Most patients describe a brief, mild pinch. The entire process usually takes no more than about 10 minutes.

Important guidance after treatment

After injection the product needs to settle in the muscle. For the best result and to reduce migration risk, follow a few simple rules:

  • Stay upright — avoid lying down or bending the head downward for about 4 hours after treatment.
  • Do not press the area — avoid rubbing, massaging, or pressure on injection sites for the first 24 hours.
  • Avoid strain and heat — skip intense exercise, saunas, and high heat on the day of treatment.

When will I see results?

Botox does not work fully on the day of treatment. The product needs time to relax the muscle.

Early change is usually seen between 3 and 5 days. The full result settles after about 14 days. If needed, a review can be scheduled about two weeks after the first treatment to confirm a precise, symmetric outcome.
